What is an Accident Report Form?
An Accident Report Form is a document that makes it possible to speed up compensation for material damages caused by traffic accidents.

If you find yourself in such a situation, it is important to complete the document correctly, write clearly (preferably in capital letters), verify all the information provided by the other party, submit evidence and, in the case of injuries, contact the police to file a report.

It is designed for accidents involving two vehicles (referred to as A and B), although it can also be used when one of the parties is a third party (such as a company or damage to the road) and when only material damage is involved. In the case of a multi-vehicle collision, the details of all involved parties must be recorded (see section 22).

It is also necessary to indicate if any damage has occurred to public property (traffic lights, signs, etc.).

Make sure you provide the following data:

Date, Time and Place

Details of the Insured Party

Include the details of the insured party, which may differ from the driver's details. Add the insured person's name, address and phone number.

Details of the Other Party

Add the details of the driver of the foreign vehicle involved in the accident. Include their name, address and phone number.

Vehicle/Trailer Information

Complete the information of the vehicles involved, including the NUMBER PLATE, make, model, and in the case of foreign vehicles, the country of registration. If possible, take a photo of the number plate for greater accuracy.

If the accident involves a foreign vehicle, it is essential to include all the relevant details of the foreign insurance company in the Accident Report Form. If the incident occurred outside the European Economic Area (EEA), make sure to include the International Motor Insurance Card (IMIC) or Green Card. If they do not have an IMIC, they may have a border insurance policy (which is essentially a temporary IMIC and equally valid).

The Accident Report Form must be signed by both drivers involved. Make sure both parties sign the document to validate its content and mutual agreement.

If there are witnesses present at the time of the accident, it is important to gather their information to support your version of events. Add the name, postal address, email address and telephone number of witnesses, if any.

Circumstances (Section 12)

Tick the relevant boxes regarding the circumstances of the accident in Section 12. These boxes are used by insurance companies to determine liability for the incident.

 

Accident Sketch (Section 13)

Draw a sketch in Section 13 to represent the position of the vehicles at the time of impact. Use arrows to indicate the direction of travel of the vehicles, mention the name of the street or road and mark the points where the vehicles are damaged.
